About Us
Elevate Sports and Spine Physical Therapy is a locally-owned and operated outpatient physical therapy clinic located in Hawks Prairie in Lacey, Washington. It was founded by Dr. Zachary Hall, who has been a physical therapist in the greater Lacey/Olympia area for the last decade.
Our therapy approach utilizes a variety of techniques and treatments to Elevate our patients and return them back to their lives after injury. We believe in specialization and continuing education to remain on the cutting edge of rehabilitation and provide our patients with the most up-to-date techniques available. We strongly believe in a patient-centered approach to care.

Zachary Hall, PT, DPT, COMT, Cert. VRS
Owner/Physical Therapist
Dr. Hall is an Olympia native. He has over 12 years of physical therapy experience in outpatient orthopedics and loves treating patients across the lifespan. He has spent the majority of his career treating patients in Lacey. Dr. Hall became interested in physical therapy while recovering from his own injuries and seeing first hand how therapy can improve your overall function and life. He is dedicated to providing an individualized care plan that is unique to each patient.
Dr. Hall has an extensive sports medicine background with both running analysis and rehabilitation of throwing athletes. He has worked with both the Milwaukee Brewers and Oakland Athletics and enjoys treating athletes across all levels.
Another and more recent passion of Dr. Hall’s has been treating patients suffering from vertigo and dizziness. His most recent continuing education has focused on the diagonsis and treatment of vertigo related conditions and lead him to become a Certified Vestibular Rehabilitation Specialist.
In his free time Dr. Hall enjoys hiking, snowboarding and spending time with his partner and their two year old son.

Stephanie Sparks, PT, DPT, MLD-C, CAFS, ASTYM Cert
Physical Therapist
Dr. Sparks is a Washington transplant originally from West Jordan, UT. She has 10 years of experience in outpatient orthopedics and loves helping people heal from their injuries. Dr. Sparks became interested in physical therapy after having knee surgery at a young age. She quickly saw how physical therapy can help you heal, return to activity and allow you to pursue your passions after injury. Her physical therapy experience allowed her to return and run middle distance track and field and cross country in college. Dr. Sparks loves working with people across all walks of life and is dedicated to helping patients heal through an individualized care and treatment approach.
Dr. Sparks first love is orthopedics physical therapy, but she has become very passionate about helping women recover from breast cancer treatments. Breast cancer treatments can have many unattended side effects including pain, limited AROM, joint pain, fatigue and lymphedema that can affect women with daily life. Her goal is to improve the quality of life for all breast cancer fighters and survivors.
In her spare time you can find Dr. Sparks reading, gardening or spending time with her husband, son and fur babies (2 dogs and 1 cat).

Catherine Rodriguez, PT, DPT, CMPT
Physical Therapist
Dr. Rodriguez has spent most of her life in Washington State. As the daughter of a Coast Guard officer, she moved frequently growing up, living in California, New York, and Maryland. She attended Newport High School in Bellevue, WA, where her involvement in soccer and lacrosse sparked her interest in physical therapy. She went on to earn her Bachelor’s degree in Exercise Science from Syracuse University, where she competed as a Division I lacrosse player, before earning her Doctorate in Physical Therapy from Chapman University in Orange, CA.
With over 11 years of outpatient experience, Dr. Rodriguez enjoys treating a wide range of patients, with special interests in athletic injuries, ACL reconstruction, lumbar spondylolisthesis and stress reactions, and vestibular dysfunction. Her treatment approach combines manual therapy, therapeutic exercise, neuromuscular retraining, and patient education to restore function and help patients return to the activities they enjoy most.
Dr. Rodriguez emphasizes progressive strengthening, purposeful movement, and empowering patients with the knowledge and tools to manage their health independently. She believes that challenging patients in meaningful ways builds strength, confidence, and long-term functional success. In addition to her clinical experience, she has spent over 10 years coaching girls' club lacrosse in the Seattle area.
Outside of the clinic, Dr. Rodriguez enjoys adventuring with her husband, mountain biking, hiking, working out, traveling, and playing and coaching lacrosse.

Hannah Hawk, PT, DPT, CSCS, RYT
Physical Therapist
Dr. Hannah Hawk is a Doctor of Physical Therapy and Certified Strength and Conditioning Specialist who joined Elevate Sports and Spine Physical Therapy in October 2026. With a diverse background in outpatient orthopedics, acute care, home health, pediatrics, strength and conditioning, and yoga instruction, Hannah brings a well-rounded, patient-centered approach to rehabilitation.
Hannah is passionate about helping patients move with confidence through functional movement and purposeful exercise that translates into meaningful improvements in everyday life, work, sports, and recreation. Her treatment philosophy focuses on looking beyond symptoms to understand the whole person, incorporating each patient's unique goals, lifestyle, movement patterns, and overall well-being into an individualized plan of care.
She is dedicated to empowering patients with the knowledge, confidence, and tools to take an active role in their recovery, build long-term resilience, and achieve lasting improvements in function and quality of life.
Outside of the clinic, Hannah enjoys exploring the outdoors with her husband and dogs, pursuing photography, and spending time immersed in music and art.
